B) SUPER DELUXE ROOM

C) DELUXE ROOM
Room Charges Rs. 6000/- per day. Initial Deposit required Rs.40,000/-

D) SINGLE ROOM
Room Charges Rs. 3600/- per day. Initial Deposit required Rs. 30,000/-

E) DOUBLE BEDDED ROOM
Bed Charges Rs. 2400/- per day. Initial Deposit required Rs. 20000/-
Two Patients in an Air Conditioned Room, Common Attached bathroom cum toilet.	Separate Attendant couch and wall mounted Almirah.

F) SEMI PRIVATE(A.C.) ROOM
Bed Charges Rs. 2200/- per day. Initial Deposit required Rs. 15,000/-

Room charges are inclusive of Diet & Nursing Care for patients and are exclusive of Consultant's Fee

Change of Bed Category: If a patient wants to upgrade the category of bed, he/she will have to pay as per the charges of Higher Category from the date of admission. However, in case of shifting to the Lower Category rates applicable to the lower category would be charged only from the date of shifting. (Subject to clearance of the dues at the time of change of category.)

	Check out time:
a)	Check out time is 12:00 noon.
b)	Doctor's visit and Operation charges are levied according to the category in which patient is admitted. However, the Investigation charges are uniform irrespective of the category.
c)	All payments are to be made in cash or Visa or Master Card.
d)	Cheques are not accepted.

	Attendant Pass :
a)	One Attendant Pass is allowed per patient.
b)	Please do not give this pass to any other person for use.
c)	Return this pass to the Security/Reception deptt. at the time of discharge of the patient.
d)	No duplicate pass will be issued, if lost.
e)	The passes are not valid for entry in any I.C.U.s or Operation Theatre. Contact Reception for Special Passes for entry in I.C.U.s.

	Visiting Hours:
a)	Working days 5.00 p.m. to 7.00 p.m.
b)	Sundays & Holidays - 12 noon to 1.00 p.m. & 5.00 p.m. to 7 p.m.

Children below 12 years are not allowed in the hospital except on Sundays from 12.00 noon to 1.00 p.m. & 5.00 p.m. to 7.00 p.m.

	Bed Tea	7:00 a.m.	7:30 a.m.
	Breakfast	8:30 a.m.	9:00 a.m.
	Lunch	12:30 p.m.	1:00 p.m.
	Evening Tea	4:00 p.m.	4:30 p.m.
	Dinner	7:30 p.m.	8:00 p.m.

	Cafeteria Services : 8:00 a.m. to 10:00 p.m.
a)	Available to the patients, their attendants visitors on cash payment a Ground Floor.
b)	Round the clock Canteen Services are available near the Car Parking Area.

	Parking Services : Round the clock - 24 hours
	Special Multiple Entry Pass for 24 hours is also available.

	Valuables :
a)	Please do not bring any valuables in the Hospital. Hospital Management shall not be responsible for any loss of your valuables either kept in your parked vehicle or in the room/bed allotted to you.
b)	Please do not handover your mobile/valuable to any unknown person or Hospital Staff.
c)	Attendants in Single Deluxe Rooms are advised not to leave the room unattended. However, if you are leaving the room alongwith the patient for investigations, please inform the concerned Sister-incharge or Staff Nurse on-duty for making arrangements to lock your room.

	Billing & Discharge:
a)	Payments are to be made in advance and whenever Intimation for advance deposit is given to you, please clear the same.
b)	For any problem regarding Billing, pleasecontact Billing Incharge - Extn. No. 162 or the Accounts Officer at Extn. No. 113.
c)	When a consultant advises Discharge of the patient, it takes some time to prepare the Final Bill Discharge Slip. As soon as the final bill is ready, you will be called to the Billing counter. Please bear with us till then.

	Ambulance Service : Round the clock - 24 hrs.
	Available on the rates as prescribed by the hospital for transporting the patients. Please contact Enquiry Window personally or at Extn. No. 161 for availing Ambulance Services.

Pharmacy : Round the clock at Ground Floor - (Extn. No. 151)

Blood Bank : Round the clock in Basement (Extn. No. 248)

	Waste Disposal :
a)	Black bag - For general, domestic and kitchen wastes etc.
b)	Red bag - For plastic tubings, syringes & other plastic items
c)	Yellow bag - Only for infectious wastes such as soiled dressing, cotton etc.
d)	Blue bag - needles, blades, broken glass & other sharp items etc.

	Donations :
a)	You are requested to not give any tips to the hospital employees.
b)	If you desire, you can make a donation. The donations to this hospital are eligible for tax relief under section 80(G) of the Income Tax Act. The Accounts Officer at Extn. No. 113 may be contacted for such donations and issue of the receipts.
c)	Donations can also be deposited in the Donation Box kept near the Mandir ad at each & every ward of this hospital.

	Confidential Suggestion Forms :
	Sky is the limit for improvement. You are requested to fill-up the Confidential Suggestion Forms available at Nursing Station/Reception. You can also give your candid observation about the service rendered by the hospital and please put it in the Suggestion box kept near Nursing Stations in Wards or Reception. Your suggestions will go a long way in improving our services further.

Kindly Contact Help Desk at Reception for any kind of help pertaining to Corporate/Panel Patients, Preventive Health Check-up schemes and other Medical Beneficial schemes provided by this hospital.

	Important things to note :-
a)	Please remember : Less attendants - Less strain on the patients.
b)	Attendants are strictly prohibited from sitting on patient's bed.
c)	Smoking & Drinking (alcohol) in the hospital are criminal offences and as such are strictly prochibited.
d)	Silence is solicited, in your own as well as other patient's interest.
e)	Use of electrical appliances other than hospital equipment is not allowed and Washing of clothes is strictly prohibited.
f)	Please conserve water & electricity.
g)	Right of admission is reserved.
h)	Please donate unused medicines for poor patients
